How Healthier at Home™ Works

Implementing Healthier at Home is a simple five-step process that requires minimal resources and integrates with your existing operations.

1. Consultation and Agreement

As a first step, we’ll work with you to understand your practice and your patients. We’ll meet to review the remote patient monitoring process and the details of what you can expect in your partnership with AET. We’ll answer any questions you have, introduce you to the team you’ll be working with, meet your team, and put an agreement in place that formally establishes our partnership. 

2. Patient Identification

Remote patient monitoring was formerly reserved exclusively for patients with chronic conditions, but now providers can offer it to patients with acute conditions as well. We commonly work with providers to monitor patients with:

  • Cardiovascular health issues
  • Hypertension
  • COPD
  • Diabetes
  • Weight management issues
  • Multiple chronic conditions

In monitoring these conditions, we can measure:

  • Blood pressure
  • Heart rate
  • Blood oxygen 
  • Weight
  • Blood glucose

If you have a specific physiological parameter you would like to measure that is not included in that list we may be able to accommodate your needs.

As part of the process, we work with you to develop inclusion/exclusion criteria to help choose which patients will benefit most with Healthier at Home monitoring. We also share our clinical protocols for monitoring and allow you to “customize” normal ranges for your practice and/or for a specific patient.

3. Patient Onboarding

Once you have identified patients who would benefit from RPM, simply FAX us an order form, we do the rest. Our team coordinates patient consent, delivery of devices and patient training. We use cellular embedded technology to make the process as simple as possible for your patients; once they step on the scale, complete their blood pressure test, or measure their heart rate with our simple devices, their reading is automatically uploaded to the cloud and requires no further action from them. No smart phone, computer or Wifi required. Our qualified staff reviews individual readings several times daily performs call backs for out-of-range or missing results and notifies your designated staff when intervention is appropriate.

4. Monitoring and Reporting

Our team monitors daily readings and contacts patients when their results are out-of-range, or when they forget to perform a test. We understand the importance of compliance and how difficult it can be for a patient to self-assess every day so we provide ongoing encouragement to patients via text message, email, and phone. When results are consistently out-of-range, we alert your designated staff member so you can intervene and escalate care to a physician or triage nurse as appropriate. All interactions and communications with patients are documented and are available for your viewing at any time through a secure log-in to our web-based clinical portal. We also provide health status reports and billing reports on your patients at the frequencies you desire.

The monitoring devices we provide your patients are easy to use, but in the event your patient needs assistance, we are ready to help. Our technical support team members are highly trained, and patient focused. They offer an empathetic, personal touch to assist our customers and your patients with all aspects of our services.

5. Billing and Revenue

In our experience, our partners see a net financial gain early in our partnership. We design our process to be low cost – in both financial and human resources. Since our team guides the process and takes on the bulk of the legwork, you can anticipate a return on your time investment very quickly. Our goal is to provide you better data on your patients with no additional burden on you or your team.

We offer two billing options; we can provide you with the documentation you need to bill on your own, or we can manage the billing process entirely and send you a check every month.
